Eve’s Place Youth Program Permission Slip
Thank you for enrolling your child in the Eve’s Place Youth Program. This program will discuss with your child the importance of healthy relationships, communication, bullying, life skills, and self-esteem. Your child will learn valuable life skills, and coping methods so they can better deal with difficult situations they may face. This program offers both individual sessions for your child, and group sessions that will work on team building skills.
The child/advocate relationship is confidential, and information disclosed to the Eve’s Place staff will remain private unless there is a disclosure of abuse, neglect, and a danger to themselves or others. Eve’s Place employees are mandated reporters in the state of Arizona, and all information will be reported to the proper authorities.
Parent/Legal Guardian Waiver
By signing the attached document, I indicate that I fully understand that Eve’s Place, and/or its’ sponsoring agency, are not responsible, except as may be required by operation of law, for any illness, injury, or mischance occurring as a result of participation in our program. I am fully responsible for any expenses medical or otherwise which may result. I am also granting permission to Eve’s Place staff, if required by emergency, to arrange for the treatment of my child without prior approval. Reasonable efforts will be made to contact me and/or obtain approval prior to invoking this decision.
I release, and hold harmless Eve’s Place, its’ employees, agents, and representatives from liability for any injury to my children and/or myself, and from liability, and responsibility for the loss or damage to my property during my participation in the program.
